Should You Keep Taking Prenatal Vitamins After Birth? A Dietitian Explains

Postpartum nutrient needs change, especially during breastfeeding. Here is how to decide whether to continue your prenatal, switch to a postnatal vitamin or use a different supplement plan.

Prenatal vitamins rarely come with instructions for what to do once the baby is born.

Some clinicians recommend continuing the same prenatal throughout breastfeeding. Supplement brands may tell you to switch immediately to a postnatal formula or keep taking their prenatal for a specific number of months. You may also be ready to stop taking it altogether, especially if swallowing it became a daily struggle during pregnancy.

So, should you keep taking prenatal vitamins after birth?

For many people, continuing a prenatal during early postpartum is reasonable. If you are breastfeeding, a prenatal or another appropriately formulated multivitamin may help cover nutrients with higher lactation requirements. However, there is no universal six-week, six-month or one-year stopping point.

The answer depends on whether you are breastfeeding, your diet, iron status, medical history and whether another pregnancy is possible in the near future.

The short answer

If you are breastfeeding, you may benefit from continuing a prenatal or switching to a multivitamin formulated for lactation. The Centers for Disease Control and Prevention does not say that every breastfeeding parent must take the same prenatal for the entire time they lactate. Its guidance states that some may benefit from a multivitamin and that health care providers should help determine which supplements are appropriate.

If you are not breastfeeding, you may still be advised to continue your prenatal temporarily. This may apply if you experienced significant blood loss, have low iron or another nutrient deficiency, eat a limited diet or plan to become pregnant again soon.

Other people may eventually switch to a standard multivitamin or stop taking a multivitamin. The decision should be based on individual needs, not the word “prenatal” or “postnatal” on the bottle.

Pregnancy and breastfeeding do not have identical nutrient needs

A prenatal is formulated around pregnancy requirements. After delivery, some nutrient needs decrease while others increase.

For adults ages 19 to 50, several key recommendations change as follows:

These reference values illustrate why the same formula will not be a perfect fit for every stage.

Iron needs are lower during lactation at the population level, but a person recovering from iron deficiency or substantial blood loss may need more than the standard recommendation. Choline and iodine needs increase because maternal intake can influence the amounts available in breast milk.

The numbers on a label still need to be interpreted alongside food intake, lab results and medical history.

Prenatal versus postnatal vitamins

“Prenatal” and “postnatal” are product categories, not guarantees about what is inside the bottle.

A postnatal vitamin may contain:

  • Less iron than a prenatal
  • More choline
  • More iodine
  • DHA or another omega-3 fat
  • Additional vitamin D or B12
  • Herbs marketed for milk production
  • Ingredients marketed for hair, mood or energy

It may also contain nearly the same formula as the brand’s prenatal.

I would not switch products based on the name alone. I would place the Supplement Facts panels next to each other and compare the serving size, nutrient amounts, added ingredients and testing information.

Iron needs should be based on your history and labs

Iron is one of the main reasons a prenatal may or may not remain appropriate after birth.

The iron RDA for pregnancy is 27 mg. During lactation, it drops to 9 mg for adults ages 19 to 50, according to the National Institutes of Health. Many prenatals continue to provide approximately 27 mg.

That amount may be useful if you entered delivery with low ferritin, experienced substantial blood loss or were diagnosed with postpartum anemia. It may also contribute to nausea, stomach pain or constipation in someone who no longer needs a pregnancy-level dose.

A lower lactation RDA should not be interpreted as an instruction to stop prescribed iron immediately after delivery. Likewise, having recently given birth is not a reason to take high-dose iron indefinitely.

Because my own ferritin dropped significantly during pregnancy, I plan to follow up on my labs after delivery. I will make my iron decision from those results instead of assuming the same dose will remain appropriate.

Choline needs increase during breastfeeding

Choline supports cell structure and nervous system function. The adequate intake increases from 450 mg during pregnancy to 550 mg during lactation.

Unfortunately, many prenatals contain very little choline or leave it out entirely. A postnatal formula may advertise choline on the front of the bottle while still providing only a small portion of the daily recommendation.

Check the actual milligram amount.

Food sources include:

  • Eggs
  • Meat and poultry
  • Fish
  • Dairy products
  • Soy foods
  • Beans
  • Some nuts, seeds and whole grains

If you regularly eat several choline-rich foods, your supplement may not need to provide the full 550 mg. If your diet contains very little choline, a separate supplement or a different multivitamin may be useful.

Check the iodine amount

Iodine is used to produce thyroid hormones. The RDA increases from 220 mcg during pregnancy to 290 mcg during lactation.

The American Thyroid Association and American Academy of Pediatrics recommend a daily supplement containing 150 mcg of iodine for people who are pregnant, planning pregnancy or lactating. Potassium iodide is the commonly recommended form.

Not every prenatal contains iodine. Others use kelp, which can have less predictable iodine amounts.

Food sources include dairy products, seafood, eggs and iodized salt. Sea salt, kosher salt and specialty salts are not necessarily iodized.

If you have a thyroid condition, review your total iodine intake with your physician or dietitian before adding a separate supplement. High iodine intake can also affect thyroid function.

Look for DHA, but check whether it is packaged separately

DHA is an omega-3 fat involved in infant brain and eye development. Some prenatal and postnatal vitamins include it, while others package it in a separate softgel.

If a product lists fish oil, check how many milligrams of DHA it actually provides. The total amount of fish oil is not the same as the amount of DHA.

The CDC advises breastfeeding parents to eat two to three servings, or 8 to 12 ounces, of fish from the FDA’s “Best Choices” category each week. Lower-mercury options include salmon, sardines, shrimp, trout and pollock. An algae-based DHA supplement is another option for people who do not eat fish.

Whether you need a separate DHA supplement will depend partly on how often you eat seafood and whether your multivitamin already contains it.

Vitamin B12 needs closer attention with a vegan diet

Vitamin B12 is naturally present in animal foods, including meat, fish, milk and eggs.

The CDC recommends that breastfeeding parents following a strict vegetarian or vegan diet speak with a health care provider about an appropriate B12 supplement. Low maternal B12 intake can also result in low B12 intake for an exclusively breastfed infant.

B12 absorption may be reduced by certain gastrointestinal conditions, previous gastrointestinal surgery and some medications. A standard prenatal may help maintain intake, but it may not provide the dose needed to treat a deficiency.

Do not assume your prenatal replaces your baby’s vitamin D

Most prenatals and postnatals include vitamin D, but the amount varies.

Breastfed and partially breastfed infants are generally advised to receive 400 IU of vitamin D daily beginning in the first few days of life. The CDC notes that families interested in using high-dose maternal vitamin D instead should discuss the risks and benefits with a health care provider.

Taking a standard prenatal vitamin does not automatically fulfill the infant vitamin D recommendation. Your supplement plan and your baby’s supplement plan are separate decisions.

Folic acid may still be relevant after pregnancy

The need for pregnancy-level folate decreases after delivery, but folic acid may remain important if another pregnancy is possible.

The CDC recommends that all women capable of becoming pregnant consume 400 mcg of folic acid daily. Neural tube development occurs very early in pregnancy, often before someone knows they are pregnant.

If you are planning another pregnancy or could become pregnant, check whether your prenatal, postnatal or standard multivitamin contains 400 mcg of folic acid.

People who have previously had a pregnancy affected by a neural tube defect receive a different recommendation and should discuss it directly with their physician.

Be cautious with lactation herbs

Some postnatal supplements contain fenugreek, moringa, goat’s rue, fennel or other herbs marketed as galactagogues, which are substances promoted for milk production.

A multivitamin does not need to contain a lactation blend to qualify as a good postpartum supplement. These herbs do not work consistently for everyone, and safety information varies by ingredient, dose and medical history.

Herbs can cause side effects, interact with medications or affect certain medical conditions. Combining multiple lactation teas, powders, capsules and snack products can also make it difficult to know how much of each ingredient you are consuming.

The FDA regulates dietary supplements as food, not as drugs. Supplements are not reviewed for effectiveness before reaching the market.

If you are considering an herbal lactation product, look up its individual ingredients in the National Library of Medicine’s LactMed database and discuss it with an appropriate health care professional.

Milk supply concerns also warrant an assessment with a lactation professional. A supplement cannot correct an ineffective latch, inadequate milk removal or an untreated medical issue.

If you are not breastfeeding

You do not automatically need to stop your prenatal the day you give birth. You also do not necessarily need to continue it for six months or a year.

Your postpartum supplement plan may depend on:

  • Blood loss during delivery
  • Hemoglobin and ferritin results
  • Vitamin D, B12 or other known deficiencies
  • Food access and dietary variety
  • Gastrointestinal conditions
  • Medications
  • Menstrual status
  • Whether another pregnancy is possible or planned

Formula feeding does not remove the need to nourish yourself during postpartum recovery. It simply changes some of the lactation-specific considerations.

If your labs are in a healthy range and your diet meets your needs, your physician or dietitian may recommend switching to a standard multivitamin or discontinuing the prenatal. If you are still correcting iron deficiency, vitamin D deficiency or another condition, you may need a targeted supplement instead.

How long should you take prenatal vitamins after birth?

There is no single timeline that applies to everyone.

If you are breastfeeding

You may continue a prenatal or another multivitamin throughout lactation, but review whether it contains adequate choline and iodine and whether its iron dose remains appropriate.

If you are not breastfeeding

Your clinician may recommend continuing it during early recovery, particularly if you experienced significant blood loss or have an existing deficiency. After that, your diet, labs and pregnancy plans can guide the decision.

If you have low ferritin or postpartum anemia

Continue the iron plan prescribed by your medical team and ask when your bloodwork should be repeated. Do not assume that every prenatal contains enough iron to treat a deficiency.

If you follow a vegan diet

Pay close attention to B12, choline, iodine, iron, vitamin D, calcium, zinc and omega-3 fats. A prenatal or postnatal may be appropriate, but the specific label needs to be reviewed.

If another pregnancy is possible

Make sure you receive 400 mcg of folic acid daily, whether it comes from a prenatal, another multivitamin or a combination of supplements and fortified food.

My postpartum supplement checklist

Before continuing or replacing a prenatal, I would check:

  • The full serving size
  • The amount of iron
  • Recent hemoglobin and ferritin results
  • The amount and form of iodine
  • The amount of choline
  • Whether DHA is included or separate
  • Vitamin D
  • Vitamin B12, especially with a plant-based diet
  • Folic acid if another pregnancy is possible
  • Added herbs or lactation blends
  • Overlap with other supplements
  • Independent third-party testing
  • Cost per full daily serving
  • Whether the product is tolerable enough to take consistently

I would also bring every supplement to the postpartum appointment, either physically or as photos of the labels. Reviewing the complete list helps identify duplicated nutrients and unnecessary ingredients.

My own plan after delivery

My current plan is to continue my prenatal during early postpartum, then reevaluate it based on how I am feeding the baby, my diet, my iron labs and how well I tolerate the supplement.

Because I experienced low ferritin during pregnancy, I am paying particular attention to iron. I am not assuming that I will need the same dose throughout breastfeeding or that food alone will correct a deficiency.

I will also check choline, iodine and DHA instead of assuming that a product covers them because it is labeled for pregnancy or postpartum.

My postpartum grocery list includes the foods I am stocking to help support those needs without relying entirely on supplements.

Frequently asked questions

Can I take a prenatal vitamin if I am not breastfeeding?

Yes, a prenatal can generally be used after delivery even if you are not breastfeeding. Whether it remains useful depends on your nutrient needs, lab results and medical history. Its iron amount deserves particular attention.

Do prenatal vitamins increase milk supply?

Prenatal vitamins are designed to provide vitamins and minerals, not to increase milk production. Correcting a nutrient deficiency may support your overall health, but taking more prenatal vitamins will not increase supply.

Should I take a prenatal and postnatal vitamin together?

Do not combine them unless a health care provider has specifically recommended it. The formulas are likely to contain many of the same nutrients, which can result in unnecessarily high doses.

Is a postnatal vitamin better than a prenatal?

Not automatically. Some postnatal products are better aligned with lactation needs, while others contain nearly the same nutrients as a prenatal or add herbs you may not need. Compare the Supplement Facts panels.

Can I keep taking prenatal vitamins after I stop breastfeeding?

You can, but the formula may no longer match your needs. Iron is a common concern because many prenatals contain more than the standard requirement for a nonpregnant adult. If another pregnancy is possible, continue getting the CDC-recommended 400 mcg of folic acid daily.

The bottom line

Many people can continue taking a prenatal vitamin after birth, particularly during early postpartum or breastfeeding. That does not make every prenatal the right long-term choice.

Check the iron, choline, iodine, DHA, vitamin D, B12 and folic acid amounts. Consider your food intake, lab results, feeding method and plans for another pregnancy. A postnatal label is useful only if the formula fits those needs.
